In your role as Community Health Worker, you will report to the Community Program Manager.
Your responsibilities will include outreach and education targeting the general community or various groups.
Tasks include community outreach, marketing support and other interventions as identified.
DUTIES Performs outreach education as assigned.
This includes presentations to community groups and organizations.
Distributes educational materials to target populations Support marketing function by preparing, collating and gathering materials and presentations for education at meetings and special events.
Identifies community churches and organizations to provide education and outreach.
Coordinates visits and education programming; determines follow-up interventions as needed.
Schedules events and meetings.
Assures that necessary supplies are available for events, meetings and activities and coordinates events and meetings.
Collaborates with community outreach nurse as necessary to ensure program goals are on track.
Supports community outreach program with database management.
Assists in the creation or procurement of educational materials targeting various community groups.
Collaborates with various community leaders on training and outreach.
Maintains accurate and complete reports, including number and type of contacts made, and detailed demographic information for evaluation purposes.
Participates in webinars and educational training to stay current with health education.
Provides clients with advocacy and referral assistance.
Assist with grant reporting.
Assist with other duties assigned.

Estimated Salary: $20 to $28 per hour based on qualifications.
